Informed consent for the return to sports and outdoor activities, year 2026

I, by completing this registration form acting on my own behalf, hereby freely and expressly declare my will to attend and take part in the program to be held at Club La Hacienda.

I understand and accept the nature, purpose and benefits of returning to the sports and recreational activities that contribute to the integral and psychological development of every human being.

The decision to take part in sports and outdoor activities is entirely voluntary; I therefore assume any health risk that may arise for me in the course of my sports practice, and I authorize the staff of Sport Tennis SAS to make use of my personal health insurance so that I may receive care and my medical expenses may be covered should that be necessary. In addition, I release Sport Tennis SAS from liability in the event of any infection with COVID 19 or any other virus, as I understand that the infection could have occurred at any time and in any place. Likewise, I accept medical care or assistance from support staff should that be necessary during the activity, and I understand that such care will be temporary and circumstantial, limited to the time in which the activity takes place; accordingly, where necessary, immediate attention will be provided and use will be made of the health insurance, prepaid health plan or EPS with which I am affiliated.

I hereby declare and assume the risks associated with the occurrence of circumstances of act of God or force majeure, such as natural disasters, disturbances of public order, or any other event that is neither foreseeable nor controllable, during my participation in the activity. Likewise, I will be responsible for looking after my belongings at the sports academy.

Personal data protection

In accordance with the Constitución Política (the Colombian Constitution), Ley 1581 de 2012 (Colombian Data Protection Law), Decreto 1377 de 2013 (implementing decree of that law), Decreto 886 de 2014 (decree on the National Register of Databases) and other concordant rules and case law, personal data must be afforded special protection, and public and/or private entities must take care to safeguard the rights of the persons held in their databases, such as employees, clients, users, suppliers and/or collaborators, ensuring the proper handling of each database and the due control it must be subject to. Escuelas Deportivas Sport Tennis SAS, in order to comply with the constitutional mandates and the other rules laid down for the processing of personal data, recognizes that each and every holder of personal data has the right to know the information about them that appears in the databases, and likewise has the right and the duty to update their data when necessary, so that all the information contained in the databases is true and accurate.

All and each of the data contained in the databases will be put to correct use, preventing unauthorized third parties from gaining access to them, since such parties may compromise, alter, publish and/or destroy the information held there; for that reason Escuelas Deportivas Sport Tennis SAS has implemented security and access protocols for its information systems, which protect all the information contained in them.

Use of image

I hereby authorize Escuelas Deportivas Sport Tennis S.A.S. to make use of my image in audiovisual productions, videos, analog or digital photographs or by any other medium. Escuelas Deportivas Sport Tennis S.A.S. is likewise authorized to make use of the economic rights, or rights of any kind, that I hold over such images, in order to include and/or publish them on radio, in the press, on websites, on television and/or in any other media, within and outside the national territory. The use both of the images and of the rights I hold over them gives rise to no payment whatsoever, and such use is deemed to be authorized free of charge to Escuelas Deportivas Sport Tennis S.A.S., in pursuit of its corporate purpose.

This consent is given freely and voluntarily, in accordance with the applicable rules and concordant case law.
